SVVA Junior EMS Program Application


The Southampton Village Volunteer Ambulance Junior EMS Program introduces high school students to emergency medical services, public service, teamwork, leadership, and community involvement in a structured, non-emergency environment.

Applicants must be enrolled in high school and reside within the Southampton School District or Tuckahoe School District. 


Acceptance is also subject to available program capacity, staffing, and supervision. If interest exceeds the number of junior members SVVA can safely and effectively accommodate, qualified applicants may be placed on a waiting list or considered for a future enrollment period.


Regular meetings are held on the first and third Monday of each month from 5:30 PM to 7:30 PM at:

Please submit the application below. After reviewing your application, we will contact you and your parent or guardian with additional information and any required consent, medical, emergency-contact, and program forms.


In your application, please address the following:


Interest in the Program

Please explain why you are interested in joining the SVVA Junior EMS Program and what interests you about EMS, healthcare, public service, leadership, or community involvement.


Experience and Goals

Please describe any CPR, first aid, EMS, fire service, lifeguard, scouting, school club, leadership, volunteer, healthcare, or community-service experience you may have. Prior experience is not required. Please also tell us what you hope to learn or accomplish through the program.


Availability and Commitments

Please describe any recurring school, sports, employment, family, religious, transportation, or other commitments that may affect your ability to attend regularly. These commitments do not automatically disqualify an applicant.


Applicant Acknowledgment

By submitting this application, I confirm that I am currently enrolled in high school and reside within the Southampton School District or Tuckahoe School District. I confirm that my parent or legal guardian is aware that I am submitting this application.

I understand that the SVVA Junior EMS Program is a non-emergency educational and community-service program. Junior members do not respond to 911 calls, serve as ambulance crew members, or provide patient care.

I certify that the information provided in this application is accurate and complete. I understand that submitting an application does not guarantee acceptance into the program.
